PAYMENT OPTION
 
 FULL Semester Payment – One time full-tuition payment due at the point of registration. (5%discunt).     
Automatic Payment Plan – The credit card designated  will be automatically charged for one quarter of the total tuition amount immediately after the point of registration, and following on December 1 and March 1.  The designated credit card should not expire before July 2012
Installment Plan – Monthly payments of the total monthly tuition are due at the point of registration and on the 1 of every month. The parent(s) or guardian of the registered child will be responsible for ensuring that the Conservatory Office receives this payment prior to the end of each billing cycle. A late fee will be applied for all payments not received by the 3 of each billing cycle month.                      
Applicable Fees:  Late payment: $25.00 if payment is not received by the 3 of each month.

WLTHDRAWAL/REFUND POLICY:    Tuition is non-refundable. Refunds will not be made, except in case of serious illness or injury, verified by a doctor’s written certificate. The conservatory reserves the right to deny any refund request. If a student is dismissed from the conservatory at the conservatory’s discretion following a violation of the conservatory policy, no tuition will be made. All refunds will be subject to a $20 processing fee. Written notice of withdrawal or termination must be received by the conservatory no fewer than 30 days before withdrawal.
The Ballet Conservatory of Salt Lake City employs thoroughly trained instructors. Utmost care is given to all students; however, the Conservatory cannot be responsible for any injuries sustained by a student.
